How it works

Step 1. Fill out our form

Fill out our form below to request samples of eligible formulas for your patients or your practice. Samples can be sent directly to your patients’ homes or your office.

Step 2. Receive confirmation

You’ll receive a confirmation email once your sample request has been approved. We’ll follow up with tracking information once your order has been shipped.

Step 3. Get samples

You’ll typically receive your samples within 2 business days, though deliveries may require more time due to increased demand.

Kate Farms is happy to provide healthcare providers access to free samples of our medical formulas for their patients to trial prior to committing to Kate Farms. HCPs may request samples be sent directly to patients’ homes, or to a practice. Sample orders are typically delivered within two business days. You will receive an order confirmation via email once your request has been approved, and a shipping confirmation with tracking once shipped.

We are committed to ensuring our products are consistently in stock and available for patients that need them most. If the product you are interested in trialing is not available here and your patient is in critical need of supply, please contact our Customer Care team.
